Presentation Proposal

Dear Presenter

Thank you for submitting a presentation proposal for the 2008 Building Bridges Conference to be held November 16-18, 2008.

This form must be completed in a single session. To make sure you have all the requested information, please review the form in its entirety before you begin. (The form will not save your information after you have closed your browser. Also, please be aware that your network may periodically "time-out," causing loss of information. So it's best to finish within a relatively short period of time.) You will not be able to retrieve and revise your information once it has been submitted.

You should receive a copy of your submission via email. To ensure that you have a copy for your records, we recommend that you also print a copy of the form prior to submission.

If you have co-presenters, please supply their names, titles, and affiliations. As the key presenter, you will be responsible for disseminating information to co-presenters. You do not have to have a co-presenter.

If you have questions or problems with the form, please contact Nathan Wittmaier.
